Based on the hugely popular NBA Jam arcade game, NBA Jam Extreme features a new 3D engine and more lifelike player movement. Instead of the complexity of controlling a complete team of players, NBA Jam Extreme is a 2-on-2 arcade game that focuses on speed and action rather than realism. The game features digitized likenesses of NBA players and color commentary by Marv Albert during the game. Only Eastern All-Stars vs. Western All-Stars can be played in the demo. Despite offering to install DirectX, the help file advises that DirectX is not included with the demo, so DirectX must already be installed. Only one 3 minute quarter can be played in the demo.
